Swim Parents, if your child will participate in either 6th Grade Swim Gym or 7th/8th Grade Swim Athletics you must complete the following Registration.  If you have any questions regarding the registration please contact me at dave.snook@houstonisd.org.

Houston ISD Sports Physical

All 6th, 7th, & 8th Graders

I understand that my athlete will not be allowed into the pool during class unless they have completed an HISD Sports Physical.

  • Physical must be dated from 5/15/23 - 8/22/23
  • Physical must be emailed to dave.snook@houstonisd.org (Write 2023 Sports Physical in the subject line of email
  • Keep the original physical.  You will need this to register at www.registermyathlete.com
  • Please, watch the 5 minute video in the next section to ensure the sports physical is filled out correctly.  Page one of the physical must be completed/dated prior to the physical and presented to the doctor before the physical.

For a free physical, contact Bellaire ER and mention that you are an athlete that will be swimming for Pershing Middle School this year. No appointment is necessary; however, it's best to avoid busy times.

HISD Sports Physical Example and Help Video

Reasons Physical Forms are rejected by the HISD Trainers.

1) They are not able to read them due to poor quality copies submitted. (One of the most common errors)

2) They are not able to read them due to handwritten information - PLEASE TYPE if possible.  (One of the most common errors)

3) Top portion of the Medical History lacks information.  They will kick the form back if not completly filled in.  Do not skip any questions from Student Name to Emergency Contact.  Make sure all phone numbers are provided, including doctor and emergency contact.  (One of the most common errors)

4) Every box under medical history MUST be checked.  If you miss one it is an automatic kickback.  Be sure not to check both yes and no.  Believe me, it happens.  (I both skipped a box and double-checked the sample, as an example.)  Automatic kickback.  (One of the most common errors)

5) If you check the box for concussions, you MUST answer the follow-up questions: how many and when was the last time.

6) If you answer yes to #15 you MUST check the follow-up questions about the location of the injury.

7) There is a section for Females Only and a section for Males Only.  Please answer the appropriate section. Leave the other section blank.  (I have seen people answer both or answer neither.)

8) Leave the box below the Males Only section unchecked unless you plan to provide an an ECG.

9) For the box titled "Explain "Yes" answers in the box below," you must type the question number and a short statement.  For example: 13) Seasonal Alergies - Over the counter medication. (One of the most common errors)

10) Box at the bottom of the page.  "I hereby state..."  Student must sign / Parent must sign / Date (One of the most common errors)

11) Make sure you don't sign the last line.  The last line is for Dr. Shetzer and myself.

I appreciate you making sure the physical is:

  • Completed no earlier than 5/15/2023
  • Uploaded to the www.registermyathlete.com website no later than Aug 22nd.
  • Emailed physical to dave.snook@houstonisd.org no later than Aug 22nd.  (Subject of the email 2023 Sports Physical)

I know sports paperwork is a painful task.  I have over one hundred athletes in my program and spend an enormous amount of time checking, emailing, working with parents, trainers, and Register My Athlete staff.  I appreciate your help getting this submitted before our first week of school so I can focus on training our athletes.

Register My Athlete (7th/8th Graders ONLY)

Attention 7th and 8th-grade swimmers! To be assigned Athletics Swim class, please ensure you complete the registration and submit your online paperwork at www.registermyathlete.com.

  • Must be completed between Jun 15 - Aug 22
  • Athletes may not be cleared by HISD Trainers by the first day of school, however, I can check RMA paperwork.  If all paperwork is submitted the athlete will be cleared to swim.

NOTE: All paperwork must be submitted to the Register My Athlete website by August 22, 2023. Failing to provide the required documents may result in removal from the class.

Code of Conduct

Our goal is to create a positive and supportive environment for all athletes. 

  • I will treat my teammates and coaches with respect.
  • I will show good sportsmanship at all practices and meets.
  • I will dress out for all classes unless I provide a doctors note or parent email.
  • I understand that failure to dress out may result in removal from the swim program.
  • I will be a positive role model and demonstrate a strong work ethic.
  • I will respect my teammates' feelings and personal space. Any behavior that is sexist, racist, homophobic, or otherwise inappropriate will not be tolerated. 
  • I will attend all team meetings and training sessions, and meets unless excused by my coach.
  • I will show respect for all facilities and property (including locker rooms) used during practices, competitions, and team activities.
  • If I disagree with an official's call, I will discuss it with my coach and not approach the official directly.
  • I will follow all of Pershing Aquatics Program's rules and codes of conduct. I understand that if I don't, I may face disciplinary action determined by my coaches and Pershing's school administrators.

Dress Code Policy

6th, 7th, and 8th graders are required to dress out for both Swim Gym and Swim Athletics classes. Don't forget your goggles and a solid black team styled swimsuit. This is our PE uniform for swim class.

Failure to dress for swim class:

  • 1st Offense – Notification to parents via email
  • 2nd Offense – Notification to parents via email
  • Wednesday Make-up session 4:00 -4:30
  • 3rd Offense – Notification to parents via email
  • Wednesday Make-up session 4:00 -4:30
  • 4th Offense – Parent Meeting and Possible removal from Class

*** Dress Code Policy (The policy resets after the Winter Break)

If no swimsuit is brought a two-page paper on a selected swim video will be required as an alternative assessment.

Please note that not bringing goggles or a solid black swimsuit will result in a 30% grade reduction for the day.

Pershing Aquatics Program: A fee of $50, which can be paid directly to the school through www.schoolpay.com. If you need to make arrangements regarding dues or require a scholarship, simply send an email to dave.snook@houstonisd.org. 

NOTE: Do not attempt to pay this fee until Aug.  I will send an email when the window opens to collect fees.  (Thank you)

The fee covers Swim Cap, food and drink for socials/meets, and End of Year Celebration.

Team Swag

This year I will be utilizing a team store.  Parents will be able to choose from a selection of items.  The store will allow the flexibility to select as much or as little merch as fits your needs.  Parents, family, and friends are welcome to buy as well.
