Swim Parents,

I have some news about forms.  

Preparticipation Physical Examination Form - This form has been updated!  I have already changed the form on the team website - www.waterpandas.swimtopia.com. If you have printed this 2017 form please throw it away and move forward with the 2021 form.  The HISD website still has the 2017 form so this is a recent happening.

Insurance Waiver Form - This form is out of date.  The new form will not be approved by the HISD School Board until July.  So we will all have to wait for this form.  

Please send me all other forms along with the physical if possible before the end of school on June 11th.  Getting all the paperwork done for 90 kids is quite a job.  I would appreciate the help.  

Students that have not turned in all paperwork by the 2nd day of school will not be allowed in the pool.  They will be with the regular pe kids until their paperwork is complete.  I don't want this for any swimmer.  Email me any questions and/or concerns.  

Finally, please have your child finish their Bio for the website.  This area will only be available if you have a team password. The goal is for all the kids to know each other.  If they are a true team they will be successful.
