Swim Parents,

HISD has just issued a statement regarding sports paperwork for the 2021 - 2022 school year.  HISD will not be accepting any paper copies.  If you already filled out the paperwork hold on to it.  Last year they said the same thing and then required paper copies.

A couple of forms will need to be uploaded to UIL Parent Portal.  The Preparticipation Physical Evaluation (Physical) and the Parent Approval.  If you need help scanning these I can scan them for you here at school.  However, you will need to upload them to the Register My Athlete UIL website yourself. 

Once on the Parent Portal use Register My Athlete to register for an account, select Pershing MS, Select your athlete, and fill out the forms online.  Then upload the physical between now to Aug 17.
