Good morning. :) 

Medical History Forms: You must fill out the Medical History Form and present it to a doctor before getting a physical.  The doctor should use the information in their evaluation.  

Also, the physical must be on the attached HISD Physical Form. (It must be the current version, 2020, of the form) Click HERE!

I encourage you to type in the information.  If they can't read the information they will deny it.  

Common reasons for the form to be kicked back are:

1) any blank space.  (Fill in everything)

2) Box unchecked.  (Every question on Medical History must have a check Yes or No)

3) Checking both Yes and No. (It happens)

4) Electrocardiogram - Do not check this unless you are providing a Electrocardiogram.

5) Large box below Electrocardiogram - You MUST have a short response for EVERY box you checked yes.  (Most common error)

6) Student and Parent must sign and date the document.  

If you don't have access to a scanner, (Preferred), you may take a photograph of the Medical History and Physical.  If you take a photo please, ensure it is very clear to avoid having the physical kicked back.

Physicals MUST be Dated on or after May 15.
