Apologies, I was under the impression that this paperwork was handled by www.registermyathlete.com, It was not.  I need each of you to complete this waiver, print it, and have it dropped by the male coaches office or the front office and placed in my mail box TOMORROW.  It requires a Notary Stamp.  This can be done by Coach Ortiz in the male coaches office for $5.  You are not required to be there for this.  I can vouch for you.  Just attached a $5 bill to the form and drop it to me tomorrow.

If you don't have insurance including Medicaid, you may purchase HISD Insurance for $35.  In this case no paperwork is required, just drop $35 cash by my office or in an envelope.  You may also pay the $35 through www.schoolpay.com, if you choose to use schoolpay just screenshot the payment receipt and email it to me.  

I regret this last minute rush I thought this was covered through www.registermyathlete.
