🚨 Important: Athletics Physicals & Registration for Competitive Swimmers 🚨

Attention 7th and 8th Grade Competitive WaterPandas and families! 🐼💦 Before you can officially join the UIL swim team and compete this season, all athletes must complete their Athletics Physical Evaluation Form and upload it into the Register My Athlete (now called AKTIVATE) system.

What you need to do:

1. Download and print the HISD Athletics Physical Evaluation Form using the link above.
2. Take the form to your doctor or clinic and have a licensed physician complete the physical exam section.
3. Once completed, scan or take a clear photo of the form.
4. Log into AKTIVATE (formerly Register My Athlete) and upload the form to your student’s profile.
5. Confirm your registration is complete so Coach Puertas can clear you for the season.

Why this matters: No athlete can participate in UIL practices, meets, or competitions until the physical form is submitted, approved, and their registration is cleared. This is required by HISD and UIL policy for the safety of every student athlete.
